ABOUT US

Advanced healthcare is a semi-independent service for young persons aged 16+. Our Head Office is in Erith, Kent, although our provisions are located throughout England. The service is managed by a Registered Mental Health Nurse who is experienced in Primary Care, trauma informed care and has a broad knowledge in working collaboratively with young people and understanding their holistic needs.

We have trained and experienced support workers, mentors and counsellors to provide mentoring, and individual talking therapy and counselling as required.

We provide care that is tailored to meet individual needs for the following client groups: Unaccompanied Asylum-Seeking Children, Homeless Young People, Care Leavers, Young People at risk of Sexual Exploitation, Young people with mental health issues, substance misuse and alcohol use and/or dependence. We also support young people with a history of offending behaviour and complex needs – which cannot be met by the local authority.

We provide care that is tailored to individual needs for the following client groups that i.e. Unaccompanied Asylum-Seeking Children – Care Leavers – Young People at risk of Sexual Exploitation.

Young people with mental health issues, substance misuse and alcohol use/ dependence .

We also support young people with a history of offending behaviour and complex needs -which cannot be met by the local authority

OUR AIM

Our aim is to empower young people to develop and maintain self care skills, self reliance and respect. This will enable the young person to make informed, age-appropriate decisions, concerning their present and future so they can achieve a successful transition into independence and adulthood.

OUR OBJECTIVES:

● To develop person-centred pathway support packages for the young people in our care

● To promote the safety of our young people by ensuring that each young person has a holistic risk and needs assessment, which is regularly reviewed

● To enable our young people to meet their health and emotional needs.

● To promote and maintain positive relationships that benefit our young people and enable them to develop their identity, self-respect and caring for their own well-being

● To promote a culture that is conducive to learning, education, training and employment, ensuring resources and support are available and encourage our young people to gain and maintain employment, education and training

● To enable our young people to develop skills to communicate and interact with others and to integrate and contribute to the community positively

● To work in partnership with Local Authorities, parents and family members, schools, employers and healthcare professionals to ensure the young person is enabled to maximise their true potential

● To ensure high quality yet cost-effective services delivered around the needs of our young people

● To promote an anti-discriminatory culture and an atmosphere of trust and respect

● To enable young people to be empowered to make choices and decisions in all areas of their lives

● To encourage young people to participate and be involved in the delivery and development of the service

24/7 staffed Semi-Independent group homes
group housing with floating support and overnight on-site support staff
Tailor made packages of support are also provided to cater to individual needs and specific requirements for a young person.
Emergency placements.
One to one mentoring and key work support for young people up to 24 hours a week
